Posted this already under the poll thread - didn't see this one. Here are three when I DEFINITELY had to turn away:

1 The beginning of 'Wild At Heart' when Cage kills a man with his bare hands (in fact I left the cinema after half an hour, the film was so bad).

2 The brain sequence in 'Hannibal' (sick!)

3 What Uma does with Elle's eyeball in 'Kill Bill 2' - however, I forgive this because the movie was so excellent.
